Job Title : Case Manager
Job Summary :
We are seeking a compassionate, detail-oriented, and organized Case Manager to support and coordinate services for individuals in need. The Case Manager will assess client needs, develop care plans, and connect clients with appropriate services and resources. This role requires strong communication, problem-solving, and advocacy skills. The case manager will work closely with the clinical team to address individual client needs.
Key Responsibilities :
- Conduct intake and case management assessments of clients' needs, strengths, and support systems.
- Develop individualized service or treatment plans with clear goals and outcomes.
- Coordinate care and services, including medical, housing, employment, mental health, or legal support.
- Monitor and evaluate client progress, making adjustments to plans as needed.
- Serve as an advocate for clients, ensuring they receive appropriate services.
- Maintain detailed and confidential client records in compliance with agency and licensing policies and regulations.
- Collaborate with other service providers, community agencies, and professionals.
- Provide crisis intervention or support when necessary.
- Educate clients and families about available resources and services.
- Ensure compliance with relevant laws, regulations, and organizational standards.
Qualifications :
1+ years of case management or relevant experience.Strong organizational and time-management skills.Excellent interpersonal and communication abilities.Knowledge of community resources and support services.Knowledge of KIPU (preferred)Knowledge of DCF, FARR, and Joint Commission standards (preferred)Pass local background checkThis is a full time position in office, Monday through Friday
