We are looking for qualified LPNs to provide the highest quality of care for our home health patients in a professional and compassionate manner. You will use assessment tools to manage the medical needs of a patient and identify other disciplines needed to maximize the patient’s outcome. You will be a patient advocate, and will be in communication with the primary Doctor, Clinical Team Manager, and assigned Registered Nurse, as the patient’s condition requires. Qualifications :

• Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
• 1 year nursing experience required, IV certification preferred
• Ability to exercise initiative and independent judgment.
