Home Health Manager / Registered Nurse

Join our team in Elk Grove, CA as a Home Health Manager responsible for coordinating medical care and services for seniors in their homes. Our goal is to provide high-quality care with compassion to the most vulnerable members of our community.

The Home Health Manager position involves overseeing skilled home healthcare services and guiding the home health team to ensure the well-being of participants. You will be responsible for coordinating nursing, physical therapy, and occupational therapy services in participants' homes, nursing homes, and board and care facilities.

Key Responsibilities

Manage and lead the home care department, ensuring adherence to clinical policies and workflows.

Supervise the home care staff, including recruitment, training, and mentoring of team members.

Evaluate the quality of home health nursing assessments and interdisciplinary team participation.

Ensure accurate delivery and documentation of nursing interventions based on individualized care plans.

Oversee care quality in contracted nursing homes and board and care facilities.

Requirements

Active California Registered Nurse (RN) License.

Minimum of 2 years of nursing experience in home health, preferably with elderly populations.

Bachelors Degree preferred, or equivalent professional experience.

Strong knowledge of nursing principles for elderly care.

Mid-Senior level experience of 2+ to 5 years (management experience not required).
