Position Overview

WELLNESS COACHES (CASE MANAGERS)

(HILL HOUSE PROGRAM - BRONX)

SCOPE OF ROLE :

The Wellness Coach will assist agency consumers with accessing needed community services (medical, psychological, substance abuse, vocational, food, clothing, through the development of a referral network, referral and follow up

Qualifications

REQUIRED EDUCATION AND EXPERIENCE

• Bachelor’s Degree required
• Working knowledge of various computer programs including e-mail, Microsoft Word, Excel, PowerPoint and Windows Operating System
