job_description.job_card.job_descriptionProvide, coordinate, and direct home nursing care according to physician's orders based on agency policies and procedures. Supervise and evaluate Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses. Average 5-8 patients per day, requiring travel from patient home to patient home (mileage reimbursed). Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. Complete all documentation within 24 hours of patient appointments. Attend periodic office meetings. Possibility to float to nearby clinics if patient census is low. Use of Point Care EMR system. Must be proficient in HCHB and OASIS and have used them within the last 6 months, or must complete a retraining course. Local candidates accepted at the same rate. Must be open to on-call rotations based on location and profession.
