RN Case Manager, Home Health

• Schedule : 40 hours / week
• Status : Full-Time Benefits Eligible
• Pay Range : $33.12 / hr - $49.68 / hr
• Location : Ideal candidate is willing flexible with county for patient coverage

The RN Case Manager provides compassionate, culturally sensitive, holistic nursing care to patients in a clinical setting, supporting the Mercyhealth nursing mission, practice values and strategic plan. May include variable shifts, weekends, holiday and / or on-call hours. Performs other duties as assigned including floating to various units based on identified needs and clinical competency.

Job Duties :

Requirements :

Graduate of an accredited nursing program.

ADN accepted, BSN Preferred

Licensed as an RN in state of practice

Approved BLS certification required within 90 days

Maintain a valid driver's license and reliable transportation with proof of vehicle insurance when requested.

Have and maintain an acceptable driving record per Mercyhealth's HR.100 Fleet and Driver Safety policy.
