Position Summary

We’re looking for a driven Account Executive to lead outreach, education, and referral development across the broader Human Services network. This role focuses on promoting our home supportive services to case managers, social workers, discharge planners, community partners, and individuals in need of in-home assistance with activities of daily living (ADLs). Your mission : expand referrals, strengthen relationships, and ensure more individuals receive the support they need to thrive safely at home.

Key Responsibilities

• Develop and maintain strong relationships with Human Services case managers, community care coordinators, social service agencies, and other referral partners.
• Serve as a trusted liaison, educating community professionals and families on home supportive services, eligibility, and the benefits of in-home care.
• Identify and cultivate new referral sources within Human Services organizations to increase awareness and utilization of services.
• Meet directly with individuals and families to explain services, answer questions, and support the intake or referral process.
• Partner with internal operations teams to ensure smooth onboarding and high satisfaction for all referred clients.
• Attend Human Services–related events, community resource fairs, and outreach programs to promote services and expand influence.
• Maintain accurate, timely documentation of activities, referrals, and communications using CRM and reporting tools.
• Track and report progress toward monthly and quarterly referral and revenue goals.
