A rapidly growing home healthcare provider is seeking a motivated and compassionate Patient Coordinator to support patient relations satisfaction and retention. This role involves building strong trusting relationships with patients and caregivers while helping ensure they receive consistent high-quality care. The ideal candidate is empathetic proactive and highly communicative thriving in a role that combines customer service follow-up and problem-solving.
Responsibilities Include :
Conducting follow-up calls to patients and caregivers
Addressing service concerns and resolving issues promptly
Building ongoing relationships to support patient retention
Coordinating with internal teams to ensure seamless care delivery
Tracking patient interactions and documenting communication
Assisting with general patient support as needed
Ideal Qualifications :
Strong communication and interpersonal skills
Empathetic patient-focused and professional
Detail-oriented and organized
Experience in healthcare coordination or customer service preferred
Home care experience and bilingual English / Spanish are strong pluses
