Coordinates and schedules field staff assignments, making certain that all patients are assigned to staff that are qualified to provide the level of care, as determined by the RN or PT assessment and with consistency to physician orders.
Communicates directly with field staff daily and documents confirmation and / or changes scheduling software.
Schedules new cases to ensure care coordination meets agency regulations and follows up with field staff on any scheduling issues including scheduling and tracking FTF recertification for Medicare compliance.
Notifies the nursing supervisor any time an employee with the required qualifications is not available, as needed.
Serves as the liaison between referral sources and MorseLife Homecare through the referral intake process, including ongoing and immediate communication with marketers; patient arid / or patient caregivers; ordering physicians; and other home health agencies (if applicable) to obtain all required and accurate information to enter into Matrix.
Updates standard reports on a routine basis, creates new patient charts, and tracks employee training requirements.
Assists with incoming calls and document communication in the Matrix system.
Monitors process workflow tasks in Matrix related to referrals, admissions, and scheduling daily.
Maintains current client roster and patient reports for management review.
