Position Overview
This role involves coordinating services to optimize patient outcomes through assessment, evaluation, collaboration, and management of care delivery systems. The focus is on implementing cost-effective strategies that ensure high-quality care within the current healthcare environment for the designated patient population.
Key Responsibilities
- Maintain a safe and clean work environment, adhering to unit safety and infection control protocols.
- Coordinate patient services to maximize outcomes effectively.
- Assess, evaluate, and collaborate with healthcare teams and participate in service line activities.
- Manage utilization, discharge planning, and other healthcare service systems.
- Serve as a liaison and facilitator among patients, families, healthcare providers, and insurance personnel.
- Oversee direct admission processes and bed assignments for patients admitted through physician offices or transfers from other facilities (emergency department focus).
- Provide coverage for holiday shifts as required.
Qualifications
Minimum of three years clinical and case management experience in an acute care setting preferred.Familiarity with Prospective Payment System, JCAHO standards, and regulatory requirements related to Utilization Management and Discharge Planning is desirable.Strong written and verbal communication skills.Education
Bachelor's degree in nursing or a related health field preferred.Licenses
Current Registered Nurse license required.Successful candidates must pass a physical exam, drug screening, and background checks prior to employment. This position is located in a regional healthcare setting within the Pacific Southwest region of the United States.
