Field-Based Behavioral Health Advocate

As a Behavioral Health Advocate, you will play a crucial part in enhancing the quality of life for adults enrolled in the Medicaid program by helping them access the necessary community services and resources. Your proactive approach and clinical liaison skills will be vital in fostering strong connections between health plan staff and clinical networks.

This field-based position involves 80% of your time spent engaging with enrollees at various locations, including hospitals, clinical facilities, community sites, and their homes. Enjoy a balanced work schedule of Monday to Friday, from 8 AM to 5 PM, with no weekends, evenings, or holidays. Please note that residency in or near Watertown or Syracuse, NY, is required for this position.

Your Impact : As a Behavioral Health Advocate, you will :

• Connect individuals with indispensable community services.
• Support enrollees with arranging aftercare appointments.
• Follow up with members based on their individual needs.
• Identify care gaps and work through obstacles to facilitate necessary connections.
• Review daily inpatient census to monitor recent readmissions.
• Collaborate with facility treatment teams to develop effective aftercare plans.
• Manage follow-ups for high-risk enrollees through calls and documentation.
• Build strong relationships with community providers as a representative of the company.

The salary for this position ranges from $58,800 to $105,000 annually, depending on various factors including experience and qualifications.
