We are a non-profit, community-owned organization with 40+ years of experience in delivering compassionate, patient-centered care across North Dakota and Minnesota. Our team is supported by full-time, dedicated physicians and nurse practitioners who specialize in hospice and palliative medicine.

Location : Fargo, ND

Schedule : Flexible scheduling with autonomy to tailor visits to patient needs (M-F)

Team Meetings : On-site once weekly

Day-to-Day Responsibilities :

50 / 50 split between home visits and assisted living facility visits

Expectation to see 4 8 patients per day depending on acuity and travel time

Collaborate with interdisciplinary care teams

Deliver exceptional, mission-aligned care to patients in a variety of home-based settings

Requirements :

Minimum of 3 years of NP / PA experience in primary care with a focus on geriatrics

Strong preference for prior home-based care experience

Current licensure in North Dakota and Minnesota (or ability to obtain)

Active DEA license

Valid driver s license and auto insurance

Ability to travel regularly within the designated territory

Experience in hospice and / or palliative care is highly preferred
