POSITION : Health Educator

RESPONSIBLE TO : HIV Program Manager

FLSA Status : Non-Exempt

Position Summary

Under the supervision of the HIV Program Manager The Health Educator works closely with the HIV Services Department to ensure the development and expansion of the PrEP patient navigation and adherence-counseling program at Imperial Beach Community Clinic (IBCC). The Health Educator will work directly with patients in the community as well as internally at IBCC to encourage access to and coverage for PrEP medications. The Health Educator will ensure that participating clients receive relevant HIV / STI-related education and services sexual health information and navigation to health insurance if eligible. The Health Educator will also help with testing outreach and care coordination.

Key Responsibilities

Outreach and Engagement

• Conduct street-based outreach.
• Assist with off-site testing program
• Establish and maintain trusting relationships with patients to support their engagement in healthcare and supportive services.
• Provide health education and information on available clinic and community services.
