Job Title : RN Home Health

Address : Richmond, VA

Job Description

Administer skilled nursing care for clients of all ages in their place of residence.
Coordinate care with the interdisciplinary team, patient / family, and referring agency.
Assess home health and hospice patients to identify their physical, psychosocial, and environmental needs.
Develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
Educate patients and families regarding care, diet, medications, and home safety.
Supervise and provide clinical directions to home health aides to ensure quality service.
Monitor assigned cases for compliance with third-party payer requirements.

Critical Information

On-call responsibilities for patients / families as assigned.

Work schedule may include travel within a 50-mile radius.

Education / Licenses Needed

Current state license as a Registered Nurse.

Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.

CHPN certification preferred.

Proof of current CPR certification.
