Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs such as completing preliminary health assessments.

Requirements
Responsibilities / Day in the Life :
Receive new patient referrals from clientsCreate charts for all new patientsInput medical information in new chartHelp Care Coordinators / Intake Coordinators with patient needs as applicableFoster relationships with their team to ensure the CareHarmony culture is positive and supportive.May be responsible for completing telephonic health assessments or chronic care management (CCM) calls with patientsOther duties as appliesRequired Skills / Abilities :
