Our client is seeking a compassionate and skilled Occupational Therapist to join their Home Health team in Marion this role you will provide in-home Occupational Therapy services to patients as prescribed by the attending physician and collaborate closely with the care team to promote patient independence and quality of life.

Key Responsibilities

Conduct Occupational Therapy assessments to evaluate patient functional levels and establish short- and long-term therapy goals.

Develop and implement individualized treatment plans to improve or restore physical and cognitive function.

Provide instruction and guidance to patients and family members on therapeutic activities self-care techniques and adaptive strategies.

Educate and train on the use of orthotic prosthetic and assistive devices; design or modify devices as needed.

Re-evaluate patient progress and adjust treatment plans accordingly.

Maintain accurate and timely clinical documentation and progress reports.

Communicate with physicians regarding patient progress outcomes and any changes in the plan of care.

Consult and collaborate with other members of the home health team regarding patient needs techniques and equipment.

Participate in agency meetings professional development sessions and continuing education.

Schedule and Shift Details

Full-time schedule; details to be discussed during the interview process.

Travel

Local travel required within the service area.

Required Qualifications

Bachelors degree or higher in Occupational Therapy

1 year of clinical experience

Valid Occupational Therapist Registered (OTR) license in Illinois (IL)

American Heart Association Basic Life Support (AHA BLS) certification (ACLS / PALS accepted)

Valid drivers license and reliable transportation

Experience with EMR systems preferred
