Maintains timely and accurate documentation in EHRs and other required platforms. Promotes medication adherence, risk reduction, and patient engagement. Ensures compliance with all applicable local, state, and federal care delivery regulations. Demonstrates proficiency in utilizing the CCIQ platform to manage and coordinate patient care, ensuring accurate documentation, follow-up, and communication across the care team. Education : Graduation from an Accredited School of Nursing required, BSN preferred. Work Experience : Minimum 1 year of experience required. Background in case management or chronic disease care preferred. Special Training, Certification or Licensure : Current Colorado Registered Nurse (RN) License and current CPR / BLS certification required upon hire for all areas. Position Summary
The Coordinated Care Management (CCM) Specialist plays a critical role in supporting patients with chronic conditions by providing education, care coordination, and ongoing case management. This position helps bridge the gap between clinical care and patient self-management by developing personalized care plans, conducting regular follow-ups, and assisting patients in navigating the healthcare system.
