• Shift Duration : Part-Time (8-hour field days, flexible start at 7 : 00 or 8 : 00 AM)
• Estimated Pay Range : 2925-3450 per week ($105 per-assessment payment model plus travel stipend)
• Assignment Length : Ongoing PRN / Locum coverage
• \tAccepted NP specialties : AGNP (Adult Geriatric Primary Care), FNP (Family Nurse Practitioner), ENP (Emergency Nurse Practitioner), ACNP (Acute Care Nurse Practitioner)\n
• \tMinimum 1 year of independent Nurse Practitioner experience (new graduates not accepted)\n\nNurse Practitioner Responsibilities\n
• \tConduct annual in-home Health Risk Assessments for geriatric patients (1 : 1 visits, no follow-up care required)\n
• \tPerform physical exams, medication reviews, and evaluate social / environmental factors\n
• \tComplete basic labs (e.g., A1C, fit tests – no blood draws)\n
• \tDocument and submit assessments via provided tablet (same-day submission required)\n
• \tAdapt to various home environments and manage logistical unpredictability (route changes, no-shows, cancellations)\n
• \tMaintain high-quality standards and timely documentation\n
• \tCommunicate effectively with patients and the clinical support team\n
