Position Overview

We are looking for a dynamic and organized Clinical Manager to oversee clinical operations, provide leadership to field clinicians, and enhance workflow efficiency through the Home Care Home Base (HCHB) EMR system. This hybrid role includes both administrative oversight and the flexibility to perform in-home visits and participate in the on-call rotation . The Clinical Manager will also play a central role in building, recruiting, training , and leading a strong clinical team.

Key Responsibilities

• Provide clinical oversight, support, and mentorship to RNs, LPNs, therapists, and other field staff
• Facilitate daily clinical workflow using Home Care Home Base (HCHB) to ensure timely and compliant documentation
• Perform in-home visits as needed to support patient care and staff coverage
• Recruit, onboard, and train new clinical team members, fostering a culture of professionalism and excellence
• Collaborate with the Coding team to ensure accurate documentation and proper claim submission
• Lead weekly clinical team meetings , reviewing key agency performance metrics, including :
• SOC (Start of Care) timeliness
• ROC (Resumption of Care) and Recertification compliance
• STAR reports and QAPI (Quality Assurance and Performance Improvement) data
• Teaching and training updates and staff development needs
• Monitor clinical documentation and ensure compliance with agency, state, and federal guidelines
• Participate in on-call rotation and assist with care coordination during non-business hours
• Partner with QA, intake, and scheduling teams to optimize patient outcomes and field operations
