Intensive Support Specialist (Assertive Community Treatment)

Overview

The Bluebonnet Trails Community Services (BTCS) Assertive Community Treatment (ACT) Team seeks an Intensive Support Specialist to provide person-centered, intensive support services to a small caseload of adults who have current or historical substance use treatment needs and a diagnosed severe and persistent mental health disorder, who have experienced multiple hospitalizations. Services are comprehensive, integrated, community-based, and delivered by a multidisciplinary team to provide psychiatric treatment, social services, and rehabilitation.

Responsibilities

• Report directly to the Behavioral Health Services Manager and support the development and maintenance of effective, professional working relationships with stakeholders and other community service providers.
• Maintain flexible hours dependent upon the needs of the caseload and, where appropriate, option to work remotely to complete required administrative tasks or televideo services as deemed appropriate by supervisor.
• Regularly meet with individuals in their homes, in the community, hospitals, crisis units, criminal justice settings, and at BTCS clinics to provide specialized case management and rehabilitative skills training services.
• Provide services intended to meet people where they are to achieve outcomes such as stabilizing mental health symptoms, reducing risk of recidivism, increasing awareness of and participation with community supports, developing self-advocacy, and supporting participation in independent behavioral health services.
• Be an integral part of a strong team with frequent contacts to share updates on the recovery process of individuals in service.
• Receive unique and specialized training and professional development focused on services to a small caseload of individuals with multiple hospitalizations and use of a state-of-the-art electronic medical record.
• Demonstrate innovative thinking, patience, resiliency, advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning, and dual-diagnosis treatment. Ability to work independently and on a team, prioritize needs during crisis, engage in regular meetings and training, and maintain a positive attitude.
