What Youll Do

As part of Strives interdisciplinary clinical care team the Nurse Practitioner (NP) will utilize theiradvanced education skills and judgment to deliver advanced nursing carefor CKD and ESKD patients. The NP will be responsible for managing patient care by working with the Strive interdisciplinary team of social workers care coordinators pharmacists an d dietitia ns to provide best in class patient Strive NP will be anadvocate navigator trusted clinician and safety net for the patientwhile delivering exceptional clinical care building empathy and trust with patients and delivering a world class patient experience. The Strive NP will work closely with the patient s existing care team (PCP Neph Cardiologist e c t. ) to collaborate o n patient care .

This position reports to the Lead Nurse Practitioner and follows a hybrid schedule : two days per week conducting in-home patient visits throughout the Newport News VA area and three days per week providing virtual care to support patients remotely.
