Job Title- Care Management Associate

Job Location- Phoenix, AZ

Pay- $17.50 / hr W2

Job Duration - 6 months contract (This is remote however you are needed to potential come onsite in Phoenix AZ as and when required)

• Coordinate medical services to support member transitions and ensure effective use of healthcare resources.
• Promote quality and efficiency in the delivery of healthcare services.
• Create service authorizations in QNXT to maintain continuity of care for members.
• Screen incoming forms using targeted business rules to identify necessary medical services.
• Make appropriate referrals to medical staff based on identified needs.
• Coordinate required services in alignment with the member's benefit plan.
• Arrange healthcare service delivery under the guidance of a nurse or medical director.
• Help ensure services are delivered in the most appropriate setting and cost-effective manner by guiding members to in-network providers
