Position Summary :
The Family Nurse Practitioner (FNP) – Wound Care Specialist (Home-Based Care) provides advanced, patient-centered wound management to individuals in their homes. This role combines the comprehensive primary care focus of an FNP with specialized expertise in wound assessment, treatment, and prevention. The practitioner delivers high-quality wound care directly in patients’ homes, ensuring comfort, continuity, and improved healing outcomes through collaboration with home health teams, physicians, and other healthcare professionals.
Key Responsibilities :
Perform comprehensive assessments of patients with wounds, ulcers, ostomies, burns, and other complex skin conditions in the home setting.
Develop and implement individualized treatment plans, including debridement, compression therapy, negative pressure wound therapy, and advanced dressing selection at the patient’s home .
Order, interpret, and evaluate diagnostic tests relevant to wound care and underlying conditions (e.g., vascular studies, labs, imaging).
Prescribe medications, topical treatments, and adjunct therapies in accordance with state licensure and organizational protocols.
Educate patients and families on wound prevention, dressing changes, infection control, nutrition, and chronic disease management.
Coordinate care and communicate effectively with referring physicians, home health nurses, and multidisciplinary care teams to ensure seamless continuity of care.
Monitor wound progression and modify treatment plans as needed to promote optimal healing outcomes.
Accurately document assessments, interventions, and progress in the electronic health record (EHR) in a timely manner.
Participate in quality improvement initiatives, case reviews, and continuing education related to wound management and advanced practice nursing.
Serve as a clinical resource and mentor for nursing and support staff regarding wound care best practices.
Maintain compliance with infection control, safety, and regulatory standards during home visits.
Qualifications :
Education & Licensure :
Master’s or Doctorate degree in Nursing from an accredited program.
Current and unrestricted Family Nurse Practitioner (FNP) license in the state of practice.
National certification as an FNP (AANP or ANCC).
Current BLS and ACLS certification.
Certification in Wound Care (e.g., WOCN , CWCN , CWS , or equivalent) preferred or willingness to obtain within 6–12 months of hire.
Experience :
Minimum of 2 years of experience as an NP; at least 1 year in wound management, home health, or related specialty preferred.
Demonstrated proficiency in wound assessment, debridement techniques, and use of advanced wound care modalities.
Experience with home-based patient care and mobile documentation systems strongly preferred.
Skills & Competencies :
Strong clinical judgment, assessment, and diagnostic abilities.
Excellent communication and interpersonal skills for patient and family education.
Proficiency with EHR systems and mobile charting tools.
Ability to work independently while maintaining strong collaboration with interdisciplinary teams.
Compassionate, patient-focused, and detail-oriented care delivery.
Work Environment :
Field-based position providing care in patients’ homes within an assigned geographic area.
Travel required between patient homes; company vehicle or mileage reimbursement provided (if applicable).
May include rotating on-call responsibilities or weekend visits as needed.
Requires ability to lift and transport medical supplies and perform wound care procedures safely in varied home environments.
