Job Summary :
We are seeking a dedicated and compassionate Physical Therapist to join our team at Emerald Care Center Claremore , a leading skilled nursing facility committed to providing exceptional care to our residents. As a Physical Therapist, you will play a crucial role in assessing, planning, and implementing personalized rehabilitation programs designed to enhance the mobility, strength, and overall quality of life for our residents.

This position operates in a clinical healthcare environment.
Responsibilities :
- Assessment and Evaluation : Conduct comprehensive evaluations of residents to assess their physical abilities, limitations, and functional needs. Develop individualized treatment plans based on assessment findings and residents' goals.
- Treatment Planning and Implementation : Design and implement therapeutic exercise programs to improve strength, mobility, balance, and coordination. Utilize a variety of therapeutic techniques and modalities to address specific conditions and promote recovery. Educate residents and their families about exercises, techniques, and strategies for improving mobility and managing conditions.
- Documentation and Reporting : Maintain accurate and detailed documentation of assessments, treatment plans, progress notes, and discharge summaries. Monitor and report on residents' progress, adjusting treatment plans as needed to ensure optimal outcomes.
- Collaboration and Communication : Work closely with interdisciplinary teams, including physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care and ensure a holistic approach to treatment. Communicate effectively with residents and their families to provide education, support, and updates on treatment progress.
- Compliance and Safety : Adhere to facility policies, procedures, and regulatory requirements related to physical therapy practices. Ensure a safe and supportive environment for residents during therapy sessions, adhering to infection control and safety protocols.
- Professional Development : Stay current with advancements in physical therapy techniques, best practices, and relevant research. Participate in continuing education opportunities and professional development activities to enhance skills and knowledge.
Qualifications :
Education : Doctoral or Master's degree in Physical Therapy from an accredited program.Licensure : Current state licensure as a Physical Therapist.Experience : Previous experience in a skilled nursing facility or similar healthcare setting is preferred but not required.Skills : Strong clinical assessment skills, excellent interpersonal and communication skills, and the ability to work effectively as part of a team.Attributes : Compassionate, patient-centered approach with a commitment to improving residents' quality of life.Physical Requirements :
