As a Family Liaison at Horizon, you will...

• Become familiar with the SBIRT public health model.
• Attend relevant community meetings in the catchment area.
• Maintain ongoing engagement with families, especially families who are unwilling or reluctant to consent to treatment for their child.
• Enlist other supportive services for the adolescent / young adult / family that may assist to address the problem that has been identified as well as address reluctance to consent to needed treatment.
• Ensure that cultural and linguistic needs are being met on behalf of each participating family.
• Maintain continuous contact with SBIRT team, adolescents / young adults, and families so that everyone is informed about a participant's full spectrum of services.
• Ensure that all patient data and information is properly recorded, entered, and stored.
• Ensure complete participant confidentiality at all times.
• Participate in relevant trainings throughout the course of the program.

Location

This position will be located at 699 Hertel Ave, Buffalo NY.

Hours

This is a full-time position from Monday - Friday.

Compensation

Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $20.19 - $22.83. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.
