Responsibilities
The Supportive Care / Palliative Care Nurse Practitioner (SPCNP) collaborates with the HarmonyCares Medical Group staff to deliver comprehensive, high-quality palliative care services under the supervision of the Regional CMO. The SPCNP will provide patient visits to both new and established palliative care patients, coordinating their care plans with the designated team, including home visits, facility visits, and hospital consultations. Engaging in independent decision-making regarding patient healthcare concerns and goals of care is a key aspect of this role.
