Compact LPN Care Navigator

A company is looking for a Care Navigator (Remote LPN).

Key Responsibilities :

Complete phone consultations with patients providing support and education about their chronic conditions

Educate patients on health conditions and empower them to manage their chronic illnesses

Maintain accurate patient records and respond to remotely transmitted patient data

Qualifications :

Current, valid Multistate / Compact Nursing License (LPN / LVN)

3+ years of experience in primary care, cardiology, internal medicine, or chronic care management

Proficiency in using electronic health records (EHR) and care management software

Reliable internet connection and proficiency with virtual communication tools

Empathy and a patient-centered approach to care
