Your Role on the Team

Position Summary :

The Case Manager accomplishes patients' care by assessing treatment needs developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches, coordinating and providing care that is safe, timely, effective, efficient, and patient-centered.

What It Takes to be Successful

Required Qualifications :

• All external candidates must have a Bachelor of Science in Nursing
• Current California RN license
• American Heart Association BLS Certification
• Three to five years of experience in an acute care hospital setting (medical / surgical preferred)
• Previous one to two years of experience as a Case Manager
• Must be proficient in typing
• Ability to write and communicate professionally
• Must be proficient in computer skills including Microsoft Office
• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
• Must demonstrate customer service skills appropriate to the job
• Excellent written and verbal communication skills in English
• Ability to maintain a work pace appropriate to the workload
• Ability to establish and maintain effective working relationships across the Health System
