Case Manager – Contract

Phoenix, AZ

20–$23 / hr

We’re hiring a compassionate and organized Case Manager for a contract role in Phoenix. Help clients navigate healthcare and social services while making a meaningful impact.

Responsibilities :

  • Assess client needs and develop care plans
  • Coordinate services and referrals
  • Monitor progress and adjust plans as needed
  • Document interactions and maintain compliance

Requirements :

  • Experience in case management or social services
  • Strong interpersonal and problem-solving skills
  • Ability to work independently and manage caseloads
  • Make a difference in the lives of those who need it most.

    Pay Details : $20.00 to $23.00 per hour
