REGISTERED NURSE - HOME HEALTH CASE MANAGER
LOCATION : WESTERN SUBURBS WITH FOCUS ON Primary : Westchester, Broadview, Oak Brook, Downers Grove, Hinsdale, Clarendon Hills, Westmont. Backup coverage for Bellwood, Maywood, Melrose Park, Forest Park, Berkeley, Hillside, Elmhurst
HOURS : DAYS WITH ROTATION TO WKDS AND HOLIDAY S
This position is also eligible for up to a $10,000.00 sign on bonus and subject to all taxes! You must be employed in the exact position at time of payout. Internal team members are not eligible for this sign on bonus at this time.
Major Responsibilities :

Required to make visits to private residences which may not be accessible to people with disabilities.Must be able to assist with ambulation including turning and transferring patients up to 200 poundsIf position has direct patient care or direct patient contact the following lifting requirement supersedes any previous lifting requirement effective 06 / 01 / 2015. Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting / movement situations will be assessed on a case-by-case basis.COMPENSATION : $78,000.00-$117,0.00 annual base salary. This position is incentive eligible.
