ESSENTIAL DUTIES AND RESPONSIBILITIES
- Conduct clinical evaluations to determine hospice eligibility and assist with preparing patients for discharge to the home setting or a GIP bed.
- Act as an internal resource to hospital staff by providing educational training and additional support to patients and families and collaborating to decrease mortality and inappropriate re-admissions.
- Functions with a “marketing” mindset to seek opportunities to serve more people and leave no patient behind. Provide the hospital staff with in-services and education on hospice and palliative care-related matters. Increase awareness around eligibility related to hospice.
- Become involved in the hospital’s committee structure as allowed that relates to strengthening end-of-life care. Functions with a “marketing “mindset to seek opportunities to serve more people and leave no patient behind.
- Collaborates with MorseLife account executives to develop and implement plans to better serve the hospital’s end-of-life care needs.
- Collaborates with hospital staff to assess and determine hospice eligibility of potential needy patients.
- Exhibits a high level of customer service and utilizes effective communication skills to improve service focus to support referral sources.
- Supports and collaborates with MorseLife Director of Hospice Services and clinical leadership to meet census goals for the organization.
- Collaborates with MorseLife medical staff to bring a higher level of value to services delivered.
- Actively involve Hospice medical staff in hospital opportunities to advance end-of-life care issues.
- Responds promptly to evaluations or informational meetings with referral sources, patients, and families.
- Perform Hospice admissions for MorseLife Hospice when requested.
- Supports requests for assistance from MorseLife access center.
- Address the “unknown-but-likely-to-occur” events in the hospital and MorseLife Hospice.
- Performs all other duties assigned.
QUALIFICATIONS EDUCATION and / or EXPERIENCE
To perform this job successfully, an individual must be able to perform each primary function satisfactorily. The requirements listed below are representative of the knowledge, skill, and / or ability required.
Five to seven years of nursing experience of which 3 need to be in a hospice setting. Experience in Oncology and / or ICU preferred.A valid Florida nursing licensee.Excellent interpersonal and communication skills.Creative problem solver with the ability to achieve positive outcomes.Ability to work as a strong team player.
