• Schedule - Full time 8am-5pm

Job Details :

Clover Home Health is hiring a Registered Nurse (RN) to support clients within the Greater Indianapolis area and its surrounding counties. Here at Clover we do not believe it's enough just to do our job, we believe that the work we do requires HEART! Our ideal Registered Nurse candidate must have home health nursing experience. This nurse candidate needs to be proficient with OASIS charting and use of EMR systems ( AXXESS preferred). Our ideal nurse candidate will work up to 40 hours per week. The nurse will complete home health visits as needed.

Requirements :

Current nursing license required for the state of Indiana

Degree in Nursing from an accredited nursing school, university or college

Valid drivers license and automobile insurance

Current in-person CPR certification

TB test screening (past 12 months)

Two years Home Health based nursing experience (required)

Two years OASIS experience (required)

Tech savvy, able to learn and navigate several platforms. Proficient with Apple products.

Perform routine duties independent of supervision but consults with others as required

Excellent communication and listening skills

Use of AXXESS, EMR system (preferred)

Excellent time management (required), tardiness not acceptable

Responsibilities :

Complete new patient intakes

Direct patient care (SOC's, ROC; Re-Certs, Discharge, Transfers, Nurse Visits and ALL OASIS documentation)

Document complex medical information to outline and improve health results that help determine immediate and routine care for the client

See patients and clients, as needed

Work collaboratively with the DON / ADON

Communicate patient conditions and collaborate with the DON / ADON to deliver care when patient needs evolve

Work to decrease re-admissions by promoting preventative care and ensuring continuity of care

Additional duties as assigned

Job Type : Full-time

Salary paid weekly : $75,000-$85,000
