LTSS Service Care Manager - J01031

Location : Remote / Field Visits to member homes and / or facilities (Source from Corpus Christi area, if possible.) Mileage will be reimbursed.

SHIFT : 8am 5pm Monday thru Friday; No overtime required

Duration : 6 months, possible extension or conversion

Job Description :

• LVN needed to assist with follow up calls to members, MD offices, DME agencies, Provider agencies, etc.
• LVN will need to work on any assigned tasks within expected turn-around time.
• LVN will need to track productivity and turn in spreadsheet each week.
• LVN may need to perform field visits in Corpus Christi and surrounding towns, if needed to assist.
• LVN will need to fill out required forms and document assessments within turn-around time.
• LVN must have good computer skills and be able to work independently in home office, being mindful of privacy to avoid any HIPAA breech.
• LVN must have stable and dependable Wi-Fi services.
• LVN must have good verbal and written communication skills.
