job_description.job_card.job_descriptionComplete discharge planning assessments within 1 day of admission (face-to-face or phone as needed) Perform Interqual reviews Coordinate with HH, SNF, IRF, LTAC, IV infusion, and DME providers for referrals and follow-up Actively participate in daily pass downs and multidisciplinary rounds Minimum 1 year of RN Case Management experience Proficiency with : MIDAS, Meditech, Care Port Discharge, Outlook, Excel, Interqual, Word Case Management experience in large healthcare systems Familiarity with NATE and MDRs Nurses who are flexible, adaptable, and quick learners Team players who work well with others and are open to direction Clinicians who are passionate about patient care coordination and transitions of care Active RN license Occasional float to 4Tele and ICU as needed

South Padre Island | $44 / hr | Day Shift (8 : 00 AM - 4 : 30 PM)
Deans Professional Services is seeking an experienced Registered Nurse Case Manager to join our team in South Padre Island. This role offers the opportunity to work in a collaborative environment, directly supporting patients and care teams in creating safe and effective discharge plans.
