This role offers the opportunity to meaningfully impact the health and quality of life of vulnerable patient populations while working in a collaborative, interdisciplinary environment.Job Duties : - Provide primary care services for patients in skilled nursing facilities - Manage chronic and acute conditions while developing individualized care plans - Collaborate closely with interdisciplinary teams, facility staff, and clinical leadership - Lead care initiatives that support improved outcomes, patient safety, and care transitions - Maintain accurate and timely documentation in accordance with Medicare and regulatory guidelines - Serve as a clinical resource and support system within the post-acute setting Qualifications : - Minimum 1 year of clinical experience - Post-acute care experience helpful, but not required-comprehensive training and support provided - Active and unrestricted Pennsylvania medical license (or in process) - Active and unrestricted DEA / prescriptive authority (or in process) - Background in IM, FM, EM, hospital medicine, or related fields welcome - Strong commitment to high-quality, patient-centered care and leadership Compensation : - Fee-for-service (FFS) model - Expected annual earnings : $180,000-$291,000 - No cap on productivity income potential Position Highlights : - Flexible rounding schedule - Uncapped earning potential - Autonomy to structure your day while receiving robust organizational support
