Description :

As a member of the interdisciplinary team, you will contribute professional nursing knowledge and skills in the provision and management of care to patients through the application of the nursing process.

You will promote the use of evidence-based practice through the nursing process, protocols, and procedures.

You will assess the patient's status through interview, observation, and physical assessment of actual problems and outcomes.

You will develop an individualized plan of care using a collaborative approach with the patient and healthcare team.

You will use appropriate interventions identified in the plan of care to achieve expected patient outcomes.

You will systematically evaluate and document the patient's progress toward expected outcomes and goals on an ongoing basis throughout the shift.

You will modify the plan of care based on evaluation and changes in patient condition as needed.

You will complete required documentation appropriately, including patient chart and event reports.

You will demonstrate professional accountability for your practice by seeking resources and continued self-development.

You will demonstrate technical knowledge of equipment and troubleshoot when problems arise.

You will analyze clinical situations systematically and act to obtain resources or correct problems to meet or exceed expected outcomes or patient needs.

You will recognize the need for assistance and utilize resources, including consultation, in a timely manner.

You will demonstrate ownership and persistence to ensure problems are resolved in the best way possible.

You will assume assignments of non-complex and some complex patient management with some turnover in assignments.

You will consistently prioritize work based on changes in patient condition, changes in assignments, and unanticipated interruptions.

You will collaborate with the healthcare team in organizing tasks and activities for the patient's care from admission to discharge according to the plan of care to meet identified expected outcomes.

You will assess and choose teaching strategies appropriate to the patient's and family's age-specific needs.

You will take cultural, age-specific, and developmental factors into consideration when teaching patients about procedures, illnesses, and the hospital experience.

You will ensure patient safety in the hospital environment by following policies and procedures and adhering to National Patient Safety Goals.

You will consult appropriate resources within the hospital to provide specialized education to patients and family members.

You will actively participate in data collection, analysis, and problem-solving and give input on process improvements and patient satisfaction.

You will assure that care provided is consistent with standards of care, evidence-based practices, and patient safety goals.

You will work with the interdisciplinary team in identifying opportunities for improvement in patient care and patient / family satisfaction.
