We are searching for a talented RN-Case Coordinator

Hours : 24hrs

Shift : Day shift with weekend and holiday rotation; 8 : 00am - 4 : 30pm

Location : St. Luke's Hospital - New Bedford, MA

Under direction of the Manager, Case Coordination, perform all the functions of a case manager. Coordinates the care / services of assigned patients along the continuum of care. Works collaboratively with the physician(s) and multidiscipline team members both internal and external to the organization to improve patient care through effective utilization and monitoring of health care resources. Facilitates patient / family / representative achievement of desired outcomes. This position is part of core Case Management staffing. It supports utilization functions including physician to physician consultation for correct level of care determination. This position also supports denial prevention and compliance with COP requirements.

Compensation : Pay rate will be determined based on level of experience.
