• FTE Status : 1.00 FTE (40 hours / week)
• Work Schedule : Day Shift, Monday-Friday. Hours will be 8am-4 : 30pm with no weekends or holidays included

Job Description :

The RN Home Care Coordinator services as the primary point of contact for hospital-based referrals to Home Health services. This role is based on the hospital campus and focuses on facilitating smooth transitions of care from the hospital to home. This role works closely with physicians, case managers, discharge planners, patients, and families to assess eligibility for Home Health services and ensure timely and appropriate referrals.
