Job Description :
The RN Home Care Coordinator services as the primary point of contact for hospital-based referrals to Home Health services. This role is based on the hospital campus and focuses on facilitating smooth transitions of care from the hospital to home. This role works closely with physicians, case managers, discharge planners, patients, and families to assess eligibility for Home Health services and ensure timely and appropriate referrals.
The Registered Nurse (RN) is responsible and accountable for providing quality professional nursing care to patients and their families and significant others adhering to the WI Nurse Practice Act, Administrative Code of the WI Board of Nursing, and / or appropriate nursing standards governing nursing in the state of practice, the ANA Nursing Scope and Standards of Practice, the ANA Code of Ethics for Nurses and Bellin Health Policies and Procedures. The responsibility includes providing a safe environment for patients, families, staff, and visitors and working collaboratively with the multi-disciplinary health care team to provide the best outcomes for the patient. Community involvement and collaboration is encouraged.Key Responsibilities :
Serve as the on-site Home Health representative within the hospital, fostering positive relationships with referral sources and clinical partnersAssess patients for Home Health eligibility and appropriateness of services in accordance with federal, state, and agency guidelinesProvide education to patients and families regarding available Home Health services, answering questions and addressing concernsCoordinate referrals and collaborate with the Home Health intake and clinical teams to ensure timely admissions and continuity of careCommunicate effectively with physicians, discharge planners, and other members of the healthcare team to promote seamless care transitionsMaintain accurate documentation and referral records in compliance with agency policies and regulatory standardsParticipate in marketing and outreach efforts to strengthen hospital and community relationshipsQualifications :
