Job Details : Care Coordination : Extensivists act as central points of contact, coordinating care between primary care providers, specialists, social workers, and care coordinators. Outpatient Management : They provide intensive outpatient treatments, including some hospital-level services like IV medications, in settings outside of the hospital. Transition Management : A key focus is managing the patient's transition from hospital to home, aiming to prevent setbacks and reduce the risk of readmission. Comprehensive Assessment : They assess a patient's complex medical needs, including psychosocial factors and social determinants of health, to develop a unified care plan Support System : They work with a patient's family and their support system to provide resources and ensure continuous care after discharge.
