Structural Heart Coordinator - APP

Job Description

The Structural Heart Program Coordinator is responsible for overseeing all aspects of the Structural Heart program, ensuring high-quality patient care, regulatory compliance, and optimal program performance. This role serves as the central point of coordination between patients, physicians, advanced practice providers, and multidisciplinary team members. The coordinator facilitates patient evaluation, procedural planning, data management, and quality improvement initiatives.

Responsibilities

Clinical Coordination :

• Coordinate the entire Structural Heart patient journey including referral intake, screening, diagnostic testing, Heart Team review, scheduling, and post-procedural follow-up.
• Serve as the primary liaison between patients and the Structural Heart team to ensure timely communication and care coordination.
• Participate in patient and family education, setting expectations and answering questions related to the procedure and recovery process.

Qualifications

Minimum Required Experience Type : 3 years clinical experience

Minimum Required Education : Bachelor's Degree required, Advanced Practice Provider

Type : BSN or Applied Health and Sciences , Masters Degree

Required Certification / License :   : RN, NP or PA
