Concho Hearts Hospice is looking for a compassionate Licensed Medical Social Worker (LMSW or LBSW) to provide PRN hospice social services in the Corpus Christi, TX area.

What You’ll Do :

• Provide emotional support and counseling to hospice patients and families.
• Assess psychosocial needs related to terminal illness and collaborate with the RN Case Manager and interdisciplinary team.
• Develop and guide individualized plans of care.
• Connect families with community and bereavement resources.
• Help patients and caregivers navigate life’s most meaningful journey with compassion and dignity.
