JOB SUMMARY

Responsible for providing and coordinating home health patient care / services consistent with the mission, vision and goals of the Agency. Speech Pathologist (SLP) is a professional member of the home health interdisciplinary team. The SLP evaluates, identifies problems, sets goals, recommends and implements mechanisms to enhance the patient’s speech, language, hearing and swallowing ability in accordance with a written plan of care prescribed by a physician.

Must have visual and hearing acuity. Must have clear verbal communication. Must be able to read, write and speak English fluently. May be exposed to minor hazards / communicable diseases. Must provide proof of auto insurance upon hire and each renewal and maintain a safe driving record.
