The VA WNY Healthcare System (VAWNYHS) is hiring a Registered Nurse- Women's Health Care Coordinator at the Buffalo VA Medical Center. The Women's Health RN Care Coordinator (WH RN CC) works with VAWNYHS Women's Health, Primary Care and Gynecology Services to assist in the provision of gender specific services and care with an emphasis on coordination of gynecologic services and mammogram coordination and follow-up. The WH RN CC functions as a liaison and is responsible for planning, coordinating, and performing a wide range of clinical, administrative, program, and technical functions to support service-wide quality care coordination and management of gender-specific services at VAWNYHS. Principle duties include but are not limited to the following :
- Completes tracking and reporting of breast and cervical cancer screening and tracking of follow-up on findings from breast and cervical cancer screening as required by VHA
- Works with ordering Providers to ensure appropriate follow-up on abnormal mammograms and abnormal cervical cytology reports and timely treatment for breast and cervical cancer as indicated
- Ensures coordination of breast and gynecological cancer care including assisting patient with navigating appointments and services, and ensuring communication between VA and community providers
- Provides care and case management services as indicated to women enrolled at VAWNYHS
- Provides back-up coverage for Maternity Care Coordinator and Women Veteran Program Manager as needed.
At the full performance level, Nurse Care Coordinator duties include but are not limited to :
The RN Care Coordinator executes position responsibilities that demonstrate expert leadership, experience, and creative approaches in providing complex patient management.Provides patient management, care coordination, and case management in an outpatient setting.The Care Coordinator assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; and other health problems.Coordinates care with multiple providers across all levels and sites of care. Coordination also includes assuring services interface within the Veterans Affairs (VA) and community programs to provide continuity of care to patients.The Care Coordinator addresses psychosocial, as well as nursing and medical needs of patients and their families / caregivers, through participation with the interdisciplinary care team.Coordinates and expedites care from diagnosis to treatment.Analyzes and manages laboratory and radiology results and provides information to the providers.Utilizes databases to provide population management to track patient care through all episodes of care.Evaluates care and outcomes to ensure timely and appropriate provision of services.Advocates for holistic care through patient / family education and provider interaction and implements an educational plan to meet the patient care needs.The Care Coordinator collaborates in the development, implementation, evaluation, and revision of policies, procedures, and / or guidelines.Demonstrates performance and leadership that is broad enough to improve the care for a group of patients.Provides peers with formal or informal education and constructive feedback for improvement.Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care.Fosters a safe and supportive environment conducive to the professional development of health care professionals.Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.Responsible for documenting broad and complex outcomes at the program or service level, that can be demonstrated at any organizational level within a facility.VA offers a comprehensive total rewards package : VA Nurse Total Rewards
