job_description.job_card.job_descriptionHome health setting primarily serving geriatric patients Covers assigned territory within specified counties, with possible travel to surrounding counties Patient caseload based on weekly visits with a minimum of 23-24 visits per week required Typical shift is days, Monday through Friday, 8 : 00 AM to 5 : 00 PM, with no weekend requirement No on-call duties required, though may be discretionary Wound care and general home health cases are typical All charting is completed using Kinnser / Wellsky EMR on site using a provided tablet Free residential parking is available Main responsibilities include performing patient assessments and treatment for home health and wound care in geriatric populations Must be able to prioritize patient care and demonstrate timely, accurate documentation First-time travelers are welcome to apply Mileage will be reimbursed from the first patient to the last patient each day, but not to or from home Any color scrubs with closed-toe shoes are acceptable attire All requests for time off must be submitted with the application; additional requests after offer will not be accepted Contracts range from 4 to 6 months, with 6 months required for less experienced clinicians A patient care-focused, punctual, and self-starting attitude is highly valued Free parking available at residential patient sites Current active state PT license in hand at time of consideration; license required if applicable Minimum 1 year of recent home health experience and 2 years of overall clinical experience Current BLS certification required No disciplinary actions or hits on license Ability to complete all charting in the clinic using Kinnser / Wellsky EMR Must be eligible to work within a 50-mile radius of the territory if considered a local candidate All required compliance documents must be completed prior to start COVID-19 vaccination required, with declination allowed for religious reasons; flu vaccination can be declined with appropriate documentation High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
