Position Overview

We are actively seeking a full-time Home Health Physical Therapist (PT) to provide direct patient care in a home setting. This role involves assessment, treatment planning, implementation, and patient / family education to promote recovery and independence.

What You'll Do :

  • Transform Lives : Conduct thorough patient evaluations and develop individualized treatment plans aligned with physician orders.
  • Empower Recovery : Deliver hands-on therapy, including therapeutic exercises, mobility training, and assistive device instruction, to enhance patient independence.
  • Educate and Support : Provide comprehensive education to patients and families on treatment procedures and home exercise programs.
  • Drive Progress : Continuously reassess patient progress and adjust treatment plans to maximize outcomes.
  • Collaborate for Success : Work closely with physicians and a multidisciplinary team to ensure seamless, comprehensive care.
  • Optimize Home Environments : Assess home environments and recommend necessary modifications or adaptive equipment.
  • Maintain Excellence : Ensure accurate and timely documentation using Epic EMR.

Qualifications

  • Bachelor's Degree in Physical Therapy (required).
  • Active Missouri Physical Therapist license in good standing (required).
  • Valid Driver's License (required).
  • Physical Therapists with a passion for home health are encouraged to apply- training available!

    • Schedule

  • Monday - Friday (8 : 00 AM - 4 : 30 PM) with occasional weekend and holiday rotation.
  • 4-10s available!
  • Rotating weekend schedule (every 6th weekend).

    • Compensation & Benefits

  • Base salary : $65,000 - $110,000 per year (based on experience).
  • $2,500 sign-on bonus (paid out in the 2nd pay period).
  • Mileage reimbursement at the IRS rate.

    • Work Location

  • Field-based role with assigned centralized territory : Florissant, Hazelwood & Bridgeton

    • Productivity Expectations

  • 27.5 productivity points per week (average of 5.5 per day).
  • Admission with OASIS = 2.5 points
  • Admission with no OASIS = 2 points
  • Evaluations / Recertifications = 1.5 points
  • Routine visits = 1 point
