LPN- Awake Overnight

The Position : We're looking for a Licensed Practical Nurse (LPN) ! You will provide health services and direct care to individuals with disabilities living in a residential program. In addition, you will act as both an advocate and a liaison with community health care providers, monitoring medical records, and medication administration.

Schedule : Monday - Friday 11 : 00 PM - 7 : 00 AM (Awake) (40 hours)

Essential Duties and Responsibilities Include :

• Provide direct specialized nursing and other day to day care to individuals.
• Document health care services in accordance with appropriate state regulations, nursing standards, and Massachusetts LPN license requirements. Abide by all agency policies and procedures.
• Function under the supervision of the program director, registered nurse and Director of Health Care.
• Act as a liaison as needed to other community agencies and treatment providers regarding regular medical needs and care. Help other DSP staff identify appropriate community health resources.
• Provide in-service education to individuals and / or staff on health issues.
• Administer medications to assigned individuals following the Department of Public Health Medication Administration Program (MAP) policies and procedures
• Provide quality assurance reviews of medication systems and MAP compliance.
• Work collaboratively with the Program Director and registered nurse evidenced by meeting weekly and as needed for medical updates.
• Work collaboratively with the nursing / healthcare staff of Bridgewell by participation in monthly nursing staff meetings and agency-wide healthcare planning.
• Collaborate with the Program Director and Registered Nurse to assess, organize, update, and maintain health care and medication needs. This should include, but is not limited to :
