Job Details
Physical Therapist seeing patients in homes. Brooklyn Territory - Visits will be clustered
What You Will Be Doing
Develop and implement treatment plans based on physician direction
Evaluate clients' physical functioning in their homes
Collaborate with clients, families, and other professionals to coordinate therapy services
Establish rehabilitation and maintenance goals and activities
Observe, record, and report patients' reactions and progress to physicians and program personnel
Provide therapy services to adult, and geriatric populations
Ensure compliance with regulatory standards
Skills You Need
Licensed and registered Physical Therapist in New York state, or ability to obtain
Graduate from an accredited physical therapy program
Minimum of 1-year experience as a physical therapist preferred
