• Outpatient
• Hours Monday-Friday 8am-5pm with no weekends.
• 12-15 Patients Per Day with additional support providing primary care to seniors with chronic conditions.
• EMR is eClinicalWorks.
• Adults only.
• Full Support team
• May 2024 start work for at least 60 days with option to extend
