Community / hospice experience is preferred Valid license form the state of practice Possess and maintains current CPR certification

Responsibilities
As a Home Health Physical Therapist, You will : Perform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and / or function
Qualifications
Minimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
