As an Advanced Practice Nurse, you will play a key role in our clinical operations, delivering direct patient care in various settings including medical centers and nursing homes. Your responsibilities will include performing geriatric assessments, establishing patient management plans, providing health education, and collaborating with a multidisciplinary team to ensure optimal patient outcomes.

Key Responsibilities :

• Conduct thorough assessments of acute and non-acute clinical issues, documenting patient histories and physical examinations.
• Develop and implement comprehensive nursing care plans tailored to the unique needs of each patient.
• Write admission, transfer, and discharge orders while ordering and interpreting lab and diagnostic studies.
• Prescribe medications in accordance with state regulations and refer patients to specialists as needed.
• Participate actively in patient care conferences and collaborate with team members to optimize treatment plans.
• Recognize and respond to emergencies by initiating appropriate life-saving measures.
